Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a vital role on assisting the Registered Manager in expanding and improving our domiciliary care services, ensuring exceptional care standards are met. Also, the role will be essential in safeguarding the health and safety of our clients, while fostering a supportive and caring atmosphere.
Duties
* Plan and manage rota that meets client needs while effectively optimising availability of care workers, with flexibility to accommodate last-minute changes
* Build and nurture relationships with clients, their families, and other professionals involved in their care.
* Conduct client consultations, assessments, reviews, and manage care planning documentation.
* Develop, update, and maintain up-to-date records of clients and carers across the database and system
* Serve as the main point of contact for clients and Care Professionals, addressing scheduling questions, care-related issues, and service updates with empathy and professionalism
* Manage correspondence, including responding to emails and phone calls in a timely manner.
* Support, mentor, and supervise carers, identifying and addressing training needs.
* Support the delivery of high-quality care by participating in quality assurance tasks, including feedback calls, reviews, and documentation audits.
* Ensure compliance with CQC standards, internal quality requirements and Equality, Diversity, and Equal Opportunities Policy in both employment and service delivery.
* Assist with on-call responsibilities as part of a rotating schedule, handling out-of-hours requests with confidence and effective decision-making
* Perform any other duties essential to the smooth operation of the business.
Key Requirements:
* At least 3 years working experience as care coordinator in the domiciliary care sector
* Must have a valid UK Driver's licence and own car
* Flexible to travel when needed
Qualifications:
* Strong organisational, verbal, and written communication skills; able to work independently and in a team.
* Calm, resilient, and solution-focused, especially when working under pressure
* Empathetic and compassionate, committed to delivering safe, high-quality care.
* Proficient in Microsoft Office and virtual communication tools; adaptable to new technologies.
* Understands confidentiality and complies with relevant legislation.
* Flexible with travel; a healthcare or social services qualification is a plus.
Schedule:
* Monday - Friday, 9am -5pm, weekends availability on a rota basis, flexible
