Urgent Response Nurse

Urgent Response is a fast paced, interdisciplinary community team aimed at preventing unnecessary admissions to hospital and facilitating timely discharges from hospital. This is an integrated health and social care team that visits patients in their own home and completes assessments in ED creating a bridge between the hospital and community. Patients are over 18 with a range of health and social care needs.

You will play a vital role in working as part of an integrated health and social care team providing a crisis intervention and early supported discharge service for patients across the City of Southampton registered with a Southampton GP practice.

Only open to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, Isle of Wight NHS FT or Hampshire Hospitals FT


Main duties of the job

Day-to-day responsibility for a designated caseload of clients requiring nursing/ rehabilitation/care management intervention and to organise and prioritise the required programmes of treatment (cases will be reviewed periodically by a more senior member of staff).

Work within the community and in University Hospital Southampton ED as a lone practitioner managing situations of risk with telephone support. Seek support and advice with cases from more experienced colleagues when faced with matters outside sphere of responsibility, knowledge, or skills.

Obtain informed consent from client to treatment, and work within agreed guidelines for clients unable to give informed consent. Obtain consent to the sharing of information and document all consent issues in accordance with relevant Trust and Local Authority procedures.

Undertake generic health and social care assessments using the Single Assessment Process (SAP).

Undertake specific comprehensive nursing assessments for clients by using investigative and analytical skills for those with acute/complex/chronic or palliative care needs e.g., clients with multiple pathology and complex social care needs and include:

* Tissue viability; dysphagia; nutritional risk; continence assessment and promotion; setting up packages of care;
* fitting standard equipment;
* identifying moving and handling risks;
* medication management.

Person Specification


Qualifications

* Level 1 Registered Nurse
* Experience of working with vulnerable adults.
* Range of relevant healthcare experience in either hospital or community setting.
* Supervision and training of others e.g. staff, students.
* Working within a multi-disciplinary team.
* Demonstrate complex nursing skills e.g. medication management, wound care, IV therapy, palliative care mgmt.
* Demonstrate good time management skills and strategies for organising own workload, prioritising and delegating appropriately.
* Demonstrate and provide evidence of clinical development and good practice examples of how key skills are applied.
