Argyll and Bute Health and Social Care Services help people from all walks of life, as well as their families or carers. Are you interested in being part of the Community Care Team Integrated Team based at Cowal Community Hospital, Dunoon? If so this is the post for you. Our overriding aim is to support individuals to maintain their independence at home and support people to regain independence after they have left hospital.


An opportunity has arisen for 1x Band 4 Assistant AHP Practitioners to join the Adult Community Care Team to provide a service alongside the Allied Health Professionals (AHPs) in our team, mainly the Physiotherapist and Occupational Therapists (the candidate will work alongside hospital and community teams and may also support other initiatives, such as Ambulatory Care).

The role is crucial in terms of the smooth day to day running for the AHPs and you will support with their caseloads, working flexibly across all aspects of our service. The post holder will be focused on prevention of admission (AHP and Frailty at the front door approaches) and Discharge to Assess approached (enhanced reablement). You will work under the supervision of a registered Allied Health Professional and use specialist assessment and rehabilitation skills with your own caseload. The primary focus of the

We are looking for, motivated & enthusiastic individuals who will be part of a multidisciplinary team working with patients/clients within their own homes/homely setting to provide holistic assessment and care planning/delivery. The post holder needs to be flexible and able to use their own initiative as well as provide high standards of patient care in support of the team to ensure delivery of a high quality service to the local community.


Training and Qualification:

* SVQ 3, HNC/D, Professional Development Award (PDA) or equivalent skills acquired through in-service development or experience and ability to undertake qualification during the first two years of employment (distance learning)
* Current UK Driving Licence (essential) Experience: · Previous experience at relevant Senior HCSW level: Physiotherapy, Occupational Therapy, rehab nursing or relevant area of social care
