Reablement - Empowering Independence
As a result of continued expansion in the reablement service, we are seeking an Assistant Team Manager to work alongside our existing leadership team. This role involves leadership, collaboration, and making a real difference!
We are looking for a confident and passionate leader to help shape our Reablement service in Wiltshire.
Reablement is a short-term service that helps people stay independent. Led by therapists, it offers practical support, advice, and equipment to help individuals reach their goals using their strengths and community resources.
As the Assistant Team Manager, you will guide a skilled team of therapists and social care practitioners, designing creative support plans, supporting people following their discharge from hospital to reorient to their home environment and routines, which may include new adjustments, connecting people with their communities, and ensuring services are delivered safely, effectively, and in line with professional standards.
Supporting the Team Manager, you will oversee service delivery, provide support in response to queries including case scenarios, supervise staff, and promote integrated working with health partners, voluntary groups, and local services. You will also lead on quality assurance and staff development, including induction processes and performance management.

Job responsibilities
Specific duties and responsibilities:
* Deputising for the Team Manager during times of absence, ensuring oversight, coordination, and leadership of the team, providing direct line management and supervision, and reporting where required to senior leadership. Use your own initiative while working collaboratively with the Team Manager, Service Managers, and Head of Service (HOS) to successfully deliver reablement across Wiltshire.
* Participate in line management and leadership activities, including chairing meetings, planning team development, and arranging training.
* Provide formal and informal supervision and support to therapy and social care staff, addressing gaps in practice and training identified through supervision and appraisal.
* Have excellent knowledge and ability to manage complex cases, with a flexible, creative, and solution-focused approach, including rehabilitation programs and Moving and Handling solutions.
* Provide expert advice, support, and facilitate creative problem solving for team members dealing with customers with complex needs, including undertaking community visits.
* Engage in Continuing Professional Development (CPD) to embed learning and promote evidence-based practice within the service.
* Ensure quality assurance standards are met, including legislative and regulatory requirements such as the Care Act 2014 and Mental Capacity Act, and monitor standards and performance of supervisees.
* Ensure assessments are conducted to appropriate standards to help individuals achieve outcomes, and long-term care is provided in line with eligibility criteria.
* Assist the Team Manager in managing resources, including capacity, individual performance, and skill mix, to ensure work is allocated safely and risks are managed effectively for optimal outcomes.
* Assist in responding to complaints, including complex and challenging cases.
* Analyze and interpret performance data to support decision-making and contribute to performance meetings, aiming for high-quality services and continuous improvement.
* Contribute to health and safety monitoring within the team, including lone working.
* Support the development and monitoring of policies and procedures.
* Work collaboratively with partners across the council, health services, voluntary sector, and independent providers to develop local connections and ensure compliance with CQC regulations.
* Proactively apply HR processes, including recruitment, induction, performance management, and sickness management, in collaboration with the Manager.
* Respond to civil emergencies, including managing a rest centre.
