Clinical Pharmacist – Primary Care
Priory Medical Group is seeking an independent prescriber to join a dynamic prescribing team at multiple York surgeries. The role offers flexibility with a mix of clinic hours and the opportunity to work from home part‑time.
Overview
The Clinical Pharmacist will deliver routine and specialist clinics across the network, manage medication reviews, support acute and urgent requests, and contribute to the PCN audit and quality improvement programmes. The position is part of a multidisciplinary team that provides comprehensive medication management for patients in GP, community, and care home settings.
Responsibilities
* Clinical medication reviews and patient monitoring in GP surgeries and care homes.
* Urgent medication requests and same‑day medication queries.
* Specialist chronic disease clinics (e.g., HRT, anticoagulation, diabetes, heart failure, hypertension, asthma/COPD, lipid management).
* Teach and support pharmacist students and community pharmacy staff.
* Assist the Prescription Admin team with medicine reconciliation, repeat prescribing, and e‑RD processing.
* Signpost patients to community pharmacy, GPs, and wider PCN team members (mental health, social prescribing, MSK, complex care home MDT).
* Participate in PCN audits, patient safety tools, and process policy development (e.g., STOPP, PINCER).
* Implement medication changes following MHRA alerts, product withdrawals, and national guidance.
* Support clinical care through the Priory Medical Group Care Home access model and chronic disease training.
Qualifications & Requirements
* UK registration with the General Pharmaceutical Council (GPhC) and a minimum of 2 years experience as a pharmacist in a general practice setting.
* Independent prescriber qualification (desirable).
* Demonstrated knowledge of common acute and long‑term conditions encountered in primary care.
* Strong communication skills with the ability to involve patients in medication decision making and support adherence according to NICE guidelines.
* Experience with clinical audits, patient safety tools, and quality improvement processes.
* Proficiency in electronic prescribing, repeat prescribing, and e‑RD.
Salary & Contract
£47,000 to £55,000 a year, pro‑ratta full‑time equivalent. Permanent contract. Flexible working patterns are available.

Additional Information
UK professional registration required. A Disclosure and Barring Service check is mandatory. The role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. The GP and nurse led routine and urgent appointments are available throughout the week, and the group maintains strong links with Hull York Medical School and the Yorkshire and Humber Deanery.
