Main duties of the job
The post holder will assess needs and provide health promotion and illness prevention services to the local population, working within a corporate caseload and with families on children and family-centred public health issues.
The candidate must be a first-level registered nurse with a Health Visiting Degree/Diploma/Certificate and adhere to the NMC Code of Professional Conduct.
Responsibilities include:
* Managing and delegating work to nursery nurses, link workers, and support staff, including supervision and monitoring
* Planning and overseeing in-service development for staff

Job responsibilities
* Demonstrate BCHC Trust Values and the 6 Cs of nursing in all relationships, developing a workload profile for specific health needs.
* Collaborate with primary health care teams and other partners to address health needs.
* Deliver the Healthy Child Programme through compassionate assessment, care planning, and evaluation, using assessment tools aligned with service specifications.
* Develop care packages with families, monitor open files, and close them upon completion.
* Conduct health promotion and disease prevention activities, reporting on key performance indicators.
* Provide evidence-based health advice to promote behavioral change and build on family strengths.
* Follow NICE guidelines on antenatal and postnatal mental health for supporting women at risk of or experiencing postnatal depression.
* Maintain quality standards through audits and monitoring systems.
* Identify and refer vulnerable children to social care and health following safeguarding policies.
* Participate in multi-agency meetings, case conferences, and child protection plans.
* Work in partnership with families and professionals to coordinate appointments and visits.
* Maintain equipment and promote immunization programs.
* Accurately record data and participate in team meetings and policy development.
* Keep comprehensive records to NMC standards, including referral letters and court reports.
* Support families through difficult situations, such as illness, domestic violence, bereavement, and child protection.
