Are you looking for an exciting new role in a high performing and supportive team? Perhaps you are ready for your next challenge? We have a fantastic opportunity for a Registered Nurse or Paramedic to join our West Berkshire Urgent Community Response and Virtual Ward Team.
The post holder will be a key member of our multidisciplinary teams including a Consultant Geriatrician, Advanced Nurse Practitioners, Registered General Nurses, Paramedic Practitioner, and Healthcare Assistants to provide an urgent community response service to patients experiencing a health crisis. The role is based at Hillcroft House, Thatcham, with staff parking available on-site.
Key Responsibilities:

2. Undertake new patient assessments and reassessments promptly, reporting back to senior staff for further discussion or assessment.
3. Analyze signs, symptoms, laboratory tests, and other measures to formulate diagnoses.
4. Develop individualized treatment and management plans based on evidence-based practices, proactively monitor patients, and review care plans.
