3-month role with a local authority
Summary
 * This is a 3-month role with a local authority, focused on supporting care leavers in their transition to adulthood. The position is crucial in coordinating and providing essential support to ensure care leavers achieve their goals as outlined in their pathway plan.
Responsibilities
 * Provide advice and practical support to care leavers.
 * Participate in the assessment and preparation of the pathway plan, including reassessment and review post-18.
 * Design and deliver interventions and plans for young people leaving care.
 * Ensure the implementation of the pathway plan.
 * Coordinate service provision and encourage care leavers to utilize available services.
 * Monitor care leavers' progress and well-being.
 * Maintain accurate and up-to-date records of interactions and services provided, including financial support.
Experience
 * Significant experience working with young people, either in a team or with partner agencies.
 * Experience in improving outcomes for young people through support delivery.
Qualification
 * NVQ Level 3 in Children and Young People or equivalent, or willingness to achieve this within 12 months of registration.
 * Detailed knowledge of child development.
 * Understanding of confidentiality.
 * Specialist knowledge of relevant legislation, including the Children Act and other education and social care laws.
Additional Information
 * Working hours: 37 hours per week.
 * Ability to travel throughout the county, including areas not well-served by public transport.
 * Enhanced DBS is required for the role.
