Be among the first 25 applicants. Due to forthcoming retirement we are recruiting for a Practice Nurse with experience in chronic disease management and specifically diabetes.
You would ideally have experience of diabetes as well as the full range of chronic disease management; hypertension; heart failure; COPD; asthma. Hours and working patterns are flexible to suit both parties and salary would depend on skill and experience.
Responsibilities
* Long-term management of patients with chronic disease through clinic reviews.
* Reviewing patient health questionnaires and test results.
* Identifying and managing patients who would benefit from optimisation of their health through education, lifestyle change and medication.
* Core nursing skills of wound care, phlebotomy, ECG taking, injections, cervical screening are essential, although it is important to note that this is not a large part of the role as many of these core services are now provided by our CTAC colleagues.
About us
We are a GMS practice based on the east coast of Fife with approximately 9200 patients, using EMIS and DOCMAN IT systems. Our surgery is based in privately owned premises that are being renovated in the coming months. We have a friendly and dedicated reception and administration team that are very much at the heart of the surgery and are a great support to our clinicians, which currently consists of: 6 x GP Partners; 1 x Salaried GP; 1 x Advanced Nurse Practitioner; 1 x Practice Pharmacist; 3 x Practice Nurses. The Practice is also complemented by: District Nursing Team; Mental Health Triage Nurse; First Contact Physiotherapist; Midwife; Community Treatment and Care Nurse; Health Care Support Workers; Primary Care Pharmacy Team.
Qualifications and requirements
* Experience in chronic disease management and especially diabetes.
* Experience in the full range of chronic disease management including hypertension, heart failure, COPD and asthma.
* Flexibility in working hours; salary to be determined by skill and experience.
* Core nursing skills as listed above are essential.
