The tACP will develop generic and specialist knowledge and skills in Critical Care Medicine to provided healthcare autonomously and within a multi-disciplinary team with a focus on Critical Care through an established ACCP training pathway. The post holder will be a 1st level registered practitioner. They will be an existing team member working within Critical Care Outreach. They will have excellent communication skills, and be able to liaise with patients and their families in planning their healthcare needs. They will be expected to provide evidence of their ongoing continuing professional development. Whilst in training the tACP will undertake the MSc in Advanced Critical Care Practice. As well as the academic pathway the tACP will follow the three year consultant-supported ACP training program mapped against established national and regional competencies. The tACP will gain the skills & knowledge under direct consultant supervision and supervision of the Clinical Lead to undertake clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions. The post holder will demonstrate critical thinking in the safe clinical decision-making process and is required to deliver an advanced level of clinical practice within critical care. Working within local and nationally agreed protocols and guidelines, the ANP demonstrates the knowledge and skills across the Four Pillars of Advanced Practice., Practice in compliance with their respective professional bodies (NMC) Code of Professional Conduct being responsible and accountable for their decisions, actions, or omissions. Be able to demonstrate a critical understanding of their broadened level of responsibility and autonomy. This includes being critically aware of their limits of their own competence, including when working with complexity, uncertainty and incomplete information. Independently prioritise, organise and manage a highly complex patient caseload. Demonstrate a high degree of proficiency and clinical acumen in relation to decision making skills, to inform clinical reasoning approaches when dealing with differentiated and undifferentiated presentations and complex situations; synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses, treating, discharging, referring to alternative specialities or escalating cares as required. Use own professional judgement to manage risk appropriately, prioritising the safety and wellbeing of individuals, families and carers. To deliver an advanced level of clinical practice within the field of Critical Care Outreach and Hospital Out of Hours (HOOH) services as part of the 24/7 team. This autonomous role will exercise independent judgement to assess, investigate, plan, implement and evaluate clinical care of the deteriorating patient. Commit to a culture of life-long learning, maintaining a professional portfolio of evidence relating to scope of practice, service specific capabilities and competencies, relevant to the role's clinical setting, in line with National Outreach Forum (NOrF) Critical Care Outreach curriculum frameworks or credentials (including with HEE or equivalent) to be evidenced during personal development plan / appraisal. Leadership Promote team working, build rapport and collaborative working practices within the multidisciplinary team. Liaise with inter-hospital departments and personnel across organisational and professional boundaries ensuring effective communication with other disciplines and organisations. Provide highly visible and accessible professional leadership and demonstrate expert knowledge and standards of clinical practice. Act as an 123.


Essential Criteria

* NMC registered
* Registered with professional regulatory body as an Independent Prescriber
* Advanced Life Support Provider
* Experience and expertise of minimum 3 years in Critical Care Outreach
* Minimum 5 years experience within critical care

* Good understanding of Advanced Practice Role within Critical Care Outreach
* Demonstrates Clinical expertise within current role
* Demonstrates leadership within current role
* Currently working on the RBH Critical Care Outreach team

Our Critical Care Outreach Service is offering an opportunity for an experienced Critical Care Outreach Nurse within the team, to undertake a Trainee Advanced Clinical Practitioner (ACP) development programme as a Band 7. Upon completion of the three-year programme, and successful completion of competencies, you will achieve Band 8a. You will be joining an existing team of Advanced Clinical Practitioners & Clinical Nurse Specialists to deliver patient-focused programmes of care, which address the complex and changing health and wellbeing needs of the deteriorating and critically ill patients across the organisation.


ACP-CCOR Role Overview

The ACP-CCOR role is a cutting-edge role in the field of Critical Care Outreach which crosses professional boundaries, undertaking traditional medical roles and leading inter-professional practice. It is a role model of excellence in the delivery of holistic care to patients. The tACP will learn to assess and treat the many varied clinical cases; both acute and chronic. The post holder will develop critical thinking in safe clinical decision-making processes and will learn how to deliver an advanced level of clinical practice within critical care. Working within local and nationally agreed protocols and guidelines, the tACP develop the knowledge and skills required across the Four Pillars of Advanced Practice., Aligned with the Royal Berkshire NHS Foundation Trust, our vision is to work together to provide outstanding care for our community.
