We are fortunate to have a full MDT as part of our core team in Fieldbay. The team includes Advanced Practitioners trained in Manual Handling, Occupational Therapy, Physiotherapy, Positive Behavioural Management (PBM) and Fundamentals of Care. We also have Speech and Language Therapists (SALT) and access to a Consultant Psychiatrist, Neuro-Psychologist and assistant Psychologists.
Our aim is to maintain and where possible, improve the overall independence levels of the people we support, through rehabilitation (if appropriate), stimulation and social integration so everyone can live their best life. Of course, we also support individuals who are less able and ensure that everyone benefits from the highest standards of care and support during their time with us. What’s involved
* Assess each resident's physical, psychological and social needs and develop a care plan considering their lifestyle, gender, and background, involving the patient, family, and multidisciplinary team.
* Continuously evaluate care needs with other members of the nursing team and ensure that care plans and records are up-to-date and accurate. Seek continual improvement in care delivery.
* Communicate clearly about the ageing process and its effects on health and lifestyle. Provide daily visits to assess individual needs, assist service users to maximize their comfort, dignity, and independence, and create an environment supporting their physical and mental well-being.
* Lead a multidisciplinary shift team, supervising staff, including orientation of new staff, appraisal, training, development, organizing duties, and monitoring performance.
