<div><p>Horsham Collaborative PCN is looking for an experienced andmotivated Paramedic to join our team under the Additional Roles ReimbursementScheme (ARRS)</p><p>The role involves working within a multidisciplinary team todeliver high-quality, accessible patient care, helping reduce GP workload andimprove outcomes.</p><p>Responsibilities include urgent and same-day care and homevisits and managing acute and long-term conditions while working autonomouslywithin the paramedics scope of practice and under appropriate clinicalsupervision.</p><p>A Paramedic under the Additional Roles Reimbursement Schemethe PCN must ensure that the paramedic is educated to degree or diploma levelin Paramedicine or equivalent experience as a registered Paramedic with the HCPC, hascompleted their two year Consolidation of Learning period as a newly qualified paramedic;, has a further three years experience as a band 6 or equivalent paramedic; andis working towards developing masters level or equivalent capability inparamedic areas of practice and, within six months of the commencement ofreimbursement for that individual or a longer time period as has completed andbeen signed off formally within the clinical pillar competencies of theparamedic FCP AP roadmap to practice.</p><p>Thursday & Friday 20 hours between 8.00 20.00</p><p>£23.50 to £25.50 an hour Depending on Experience</p><h3>Main duties of the job</h3><p><b>Clinical Responsibilities</b></p><p>Assess diagnose treat and manage patients presenting with arange of acute urgent and long-term conditions</p><p>Undertake same-day appointments and home visits across thePCN</p><p>Work autonomously within agreed clinical protocolsescalating to GPs or senior clinicians where appropriate</p><p>Identify red flags and refer patients appropriately tosecondary care community services or emergency services</p><p>Prescribe medications where appropriately qualified andauthorised in line with local and national policy</p><p>Support admission avoidance and proactive patient management</p><p>Contribute to improved access and continuity of care withinthe PCN</p><p>Work as an integral member of the PCN multidisciplinary teamMDT including GPs nurses pharmacists and allied health professionals</p><p>Participate in MDT meetings clinical supervision and PCNforums as required</p><p>Communicate effectively with individual member practicesregarding patient care and service delivery</p><p>Deliver services across the PCN practice sites in line withagreed rotas and service models</p><p>Record all patient activity accurately and contemporaneouslyon the clinical system</p><p>Ensure all patient contacts are appropriately coded andrecorded to support ARRS reporting and funding assurance</p><p>Adhere to PCN and practice policies including lone workingsafeguarding infection control and zero tolerance</p><h3>About us</h3><p>Horsham Collaborative Primary Care Network is an NHScollaboration between 4 GP Practices Courtyard, The Village Southwater,Cowfold & Rudgwick.</p><p>Our surgery teams work closely with each other, enjoying theability to share expertise and resources to develop new services.
