Qualifications & Requirements
Social Work Qualification and registration with Social Work England.
Medical Clearance – Pre-placement health assessment (PPHA).
Essential Car Documentation – This post is a designated Essential Car User. Must hold a full valid driving licence and provide own car for use at work. Receive the Essential Car User Allowance.
DBS (Disclosure and Barring Service) clearance – Enhanced DBS check required.

Responsibilities
The Discharge to Assess Team is a multi-disciplinary team responsible for supporting timely and safe transfer of care from acute and non-acute beds when citizens are medically optimised for discharge.
The role-holder will work across the hospital setting, intermediate care units, and the community, working in partnership with health colleagues to assess and plan the transfer of care, promoting independence, ensuring a strength-based approach at all times.
In the hospital team, responsibilities include:
* Covering duty.
* Supporting the discharge team with complex discharges.
* Completing safeguarding enquiries and best interest decision making.
In the community team, responsibilities include:
* Working in an integrated team to prevent hospital admissions.
* Assessing citizens recently discharged from hospital.
* Completing Care Act strengths based and trauma informed assessments.
* Covering duty.
The role holder will be expected to work independently, using own initiative, evidencing decisions made in a timely manner. The role holder will be accountable for highly complex decision making across a range of social work duties and responsibilities.
Qualifications
General Skills – Communication, Planning & Organising, Analytical, Problem Solving & Decision Making, ICT.
Communication Skills – able to effectively transfer key and complex information to all levels of staff, adapting style, advising others and dealing with sensitive issues in difficult situations, negotiating riskier demands.
Planning and Organising Skills – excellent time management, create own work schedules, prioritise, prepare in advance, set realistic timescales, visualise sequences of actions.
Analytical Skills – absorb, understand and assimilate complex information quickly, compare information from different sources, identify patterns and trends, propose realistic conclusions with risk assessment.
Problem Solving and Decision Making Skills – formulate independently a range of options for new or unfamiliar situations and select appropriate course of action, make independent decisions of a relatively uniform nature.
ICT Skills – use ICT systems to obtain and analyse data and present effectively through variety of ICT channels.
