Overview

Job summary

This is an exciting opportunity to join us at Salisbury Medical Practice. We are looking for someone who is a qualified and competent Pharmacist in the UK and desirably has previous experience working within a General Practice environment. Applicants must have excellent organisational and communications skills as well as a high level of competency using computers. The successful candidate will have the ability to liaise with staff at all levels and work effectively as part of a team. A strong commitment to personal and professional development and teamwork are essential for this role. The applicant must be a qualified, registered independent non-medical prescriber (or working towards/intent of gaining independent prescribing qualification).

Desired hours can be discussed at interview, however the minimum number per week is 18.75.

Job responsibilities

Key Duties

* To act as the first point of contact to provide specialist expert knowledge in the use of medicines and for all prescription queries and medication requests from staff and patients.
* To undertake clinical medication reviews with patients where medicine optimisation is required to reduce waste and promote self-care.
* To reconcile discharge summaries and medications for patients being discharged from hospital and/or moving into a care setting.
* To be responsible for the care management of patients with chronic diseases and undertake medication reviews to proactively manage patients with complex poly-pharmacy (including but limited to the elderly, care home residents, co-morbidity patients etc.).
* To implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
* If an independent prescriber, to prescribe, monitor and review medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
* To undertake CPPE training as identified.
* To ensure patient confidentiality, respect, and dignity is adhered to at all times.

Person Specification

Knowledge

* Essential: Demonstrate sufficient knowledge of the English language to communicate effectively with staff and patients
* Desirable: Clinical computer systems SystmOne (TPP), Ardens Templates, Knowledge of NICE guidelines, Knowledge of QOF, An understanding of prescribing budgets and financial information, Microsoft Windows Applications Outlook

Qualifications

* Essential: Pharmacy Degree (or equivalent), Registered with the GPhC, Qualified, Registered Independent Non-Medical Prescriber or willing to attend training

Qualities

* Essential: Independent worker / Autonomous, Can work collaboratively in a team, Ability to multitask, Flexible approach to demand, Calm and professional disposition

Experience

* Essential: 2 years+ Trained Pharmacist Experience, Experience in undertaking medication reviews
* Desirable: 3 years+ Trained Pharmacist Experience, Experience working in General Practice or Primary Care

Skills

* Essential: Excellent communication skills both verbal and written, Excellent organisational skills, Ability to prioritise workload
