This role is open only to current NHS staff in Cornwall or the Isles of Scilly from the following organisations:

* Royal Cornwall Hospitals NHS Trust
* Cornwall and Isles of Scilly Integrated Care Board
The Central Integrated Transfer of Care Hub (ITOCH) team is seeking a clinical prioritiser to coordinate local demand and capacity, ensuring that individual needs are triaged, prioritised and directed to the most appropriate resource within a multidisciplinary, cross‑organisational team.
Key Responsibilities
* Triage and process referrals using the RiO and STRATA systems.
* Coordinate demand and capacity to prioritise patient needs across existing team boundaries.
* Apply specialised knowledge in frailty and intermediate care to support advanced assessment and care planning.
* Ensure effective flexibility of workforce to improve service delivery efficiency.
* Enhance communication to speed up escalation and add flexibility to response.
* Travel frequently to various county locations, including areas not accessible by public transport, and manage travel requirements.
* Collaborate with the multidisciplinary and cross‑organisational team to provide seamless pathways through acute and community services.
Qualifications & Requirements
* Band 5 staff currently employed within the above NHS organisations (encouraged to apply).
* Experience or experience‑related training in triage, demand management or related clinical roles.
* Ability to travel across Cornwall and the Isles of Scilly as required.
* Strong communication and organisational skills.
