Overview
Full time post (37.5 hours per week) but part time work will also be considered. This is a 7 day service with staff covering weekends on a rostered basis. The role involves visits to patients in their own homes. A full valid driving license and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010). Training provided for electronic patient records using SystmOne. This post attracts a High Cost Area Supplement of 5% of basic salary (pro rata where applicable). Word versions of documents are available to allow adaptations (e.g., font, colour, background, or Read Aloud).
Responsibilities
* Visit patients in their own homes as part of a community therapy team.
* Manage your own caseload with support from qualified therapists, including falls prevention, mobility assessments and rehabilitation following routine surgery.
* Provide basic equipment and walking aids as required.
* Complete electronic patient records using SystmOne (training provided).
Qualifications / Requirements
* Experience in a rehabilitation environment or as part of a multidisciplinary team is desirable.
* A health-related qualification to the level of NVQ 3 or equivalent community experience is preferred.
* Full driving license and access to a car for business purposes (subject to reasonable adjustments under the Equality Act 2010).
