We are looking for an experienced level III Hospital Social Worker to join our new ‘Discharge to Assess & Recover team’. You must be registered with ‘Social Care Wales’ and hold a clean driving licence. This post is full time 37hrs pw but we are willing to consider part time.

This team provides support to the citizens of Flintshire, in order to facilitate timely hospital discharges. The post-holder will work within a multi-disciplinary team to provide a holistic assessment in the discharge process.
Flintshire Hospital Social Work teams have access to various supporting services such as an in-house Reablement team, Community Resource team, Intermediate Care and therapeutic intervention for individuals.
This role is fast paced and you will hold the case until the person is discharged to the ‘Discharge to Assess/Intake’ team with short term intervention. Flintshire has a good track record for supporting their employee’s well-being and career development via regular supportive one to one supervision and the opportunity for peer support and training.
