Experience of having worked or supported individuals to live their lives as they wish.


Skills and Specialist Knowledge

Essential: Good interpersonal, communication and recording skills, respectful and honest.

Language Requirements

Listening and Speaking – Intermediate

* Maintain a fluent conversation on a number of everyday subjects
* Discuss cases involving the field of work
* Follow discussions in Welsh, in clear Welsh, on familiar matters relating to the post
* Contribute to the conversation and answer questions

Reading and Understanding – Intermediate

* Understand everyday correspondence on familiar issues at work
* Understand longer reports in clear Welsh and pick up the main points (may need support with vocabulary)

Writing – Intermediate

* Write letters for specific purpose, e‑mails and short reports in Welsh and English, using vocabulary and simple phrases familiar to the field of work (to be checked before sending)


Job Description

Purpose of the Post:

* Ensure that the people of Gwynedd are at the heart of everything we do.
* Help individuals to live their lives as they wish to live.
* Take on the full care role in the home.
* Collaborate with other agencies, attend reviews, meetings with health, and consult with specialists.
* Administer medicines.
* Ensure appropriate use of users’ property (medicine and cash).
* Make appropriate use of specialist equipment to assist the individual to live their life as they wish.

Main Duties – Support the Individuals Who Use the Service:

* Assume full charge of home role where appropriate.
* Lead staff with their work duties.
* Make decisions in the absence of managers (e.g., receive admissions back from hospital, lead in an emergency, respond to enquiries).
* Lead and respond to an emergency situation involving users and the building.
* Identify changes in the health and well‑being of individuals and respond quickly and appropriately.
* Make informed decisions independently, applying professional opinion and knowing when to seek wider team support.
* Build relationships with individuals to understand their needs and empower their voice.
* Provide innovative and creative care, including active and positive behavioural support.
* Maintain and support the procedure for administering and distributing medicines.
* Promote the rights of the individual and encourage them to take positive, informed risks.
* Develop personal care plans in partnership with the individual.
* Plan and implement activities in accordance with the user’s needs.
* Encourage and support users to develop social skills and community participation.
* Contribute to multidisciplinary talks on personal well‑being outcomes.
* Assist with house‑keeping duties and light meal preparation.
* Operate and maintain IT systems.

Communication and Relationships:

* Work in partnership with health and care professionals (social workers, nurses, therapists, GPs, 3rd sector partners).
* Build links to family and community networks.

Training and Supervision:

* Commit to self‑development and continuous professional development.
* Attend supervision and team meetings.
* Register with the Care Council for Wales.

Safeguarding:

* Report any concern or suspicion of abuse of a child or vulnerable adult.

Policies:

* Manage information in accordance with Council standards.
* Ensure compliance with Health and Safety regulations.
* Operate within equal opportunities and equality policies.
* Fully comply with Cyngor Gwynedd's Code of Practice for Medicines.

Other:

* Commit to reducing the Council’s carbon emissions.
* Undertake any reasonable duty corresponding to the salary and responsibility level.


Special Circumstances

* DBS checks required.
* Unsociable hours, including bank holidays and weekends on a rota basis.
* Potential to work across other service sites.
