Community Support Worker
Place Based Team – Warwick & Kenilworth: provide patient‑centred care and work closely with the multidisciplinary team to support individuals in their own homes.

Job responsibilities
* Provide high‑quality, caring and compassionate personal care to patients, following the advice of registered professionals and working independently under the approved core task list.
* Assist with assessment, treatment, investigations and procedures as directed, and support the discharge process.
* Document all care accurately and legibly, keeping patient records confidential and compliant with the Data Protection Act.
* Actively support patients and relatives by providing information, compassion and empathy, and chaperone or escort patients when necessary.
* Maintain a clean, safe and tidy clinical area, handle infection control procedures, and manage waste and linen disposal.
* Support team members and share responsibility for equipment maintenance, storage areas and hospital supplies.
* Carry out administrative tasks, such as reporting incidents, accidents and safeguarding concerns to the appropriate professional.
* Take part in team meetings, audits and appraisals, completing mandatory training (infection control, resuscitation, manual handling, IT).
Qualifications
* GCSE level A‑C in English and Maths (or equivalent).
Experience
* Experience delivering care in a healthcare or people‑centred environment.
* Team‑working experience.
Skills
* Competence in care skills.
* Strong written, verbal and non‑verbal communication skills.
* Ability to present information clearly and accurately.
* Capacity to organise and prioritise workload.
* Skill in keeping accurate, legible patient notes.
