The Network Clinical Pharmacist will work as an integral part of the Chorley Central Primary Care Network multidisciplinary team to improve the quality, safety, and cost-effectiveness of medicines use across our GP practices. This is a key clinical role within the PCN, focusing on medicines optimisation, long-term condition management, and reducing medication-related harm. Working collaboratively with GPs, nurses, pharmacy technicians, and wider healthcare professionals, the post holder will help ensure patients receive safe, effective, and evidence-based care. The post holder will contribute to the achievement of Network Contract DES and Quality and Outcomes Framework (QOF) targets, supporting the PCN to deliver high-quality, patient-centred care in line with NHS priorities. Main Duties and Responsibilities Clinical Responsibilities Conduct Structured Medication Reviews (SMRs) for patients with complex polypharmacy, frailty, or long-term conditions, ensuring safe and effective medicines use. Support medicines reconciliation for patients recently discharged from hospital or other care settings. Identify patients at high risk of adverse drug events and take proactive steps to reduce harm. Manage repeat prescribing processes and assist with acute prescription requests. Implement prescribing changes in line with national and local guidelines (NICE, LMMG, ICB). Work closely with clinicians to optimise treatment for long-term conditions such as hypertension, diabetes, and respiratory disease. Provide face-to-face and remote consultations with patients to discuss medication use, adherence, and lifestyle support. Use independent prescribing skills to manage appropriate patient cohorts within scope of practice. Medicines Optimisation and Quality Improvement Support practices to deliver safe, cost-effective prescribing and reduce unwarranted variation. Participate in clinical audits, data reviews, and quality improvement projects. Develop and implement local prescribing policies, care pathways, and protocols across the network. Contribute to QOF, Enhanced Services, and target delivery, supporting PCN performance and patient outcomes. Collaborative and System Working Build strong working relationships with community and hospital pharmacy teams to improve communication, reduce duplication, and support seamless patient care. Liaise with Integrated Care Board (ICB) Medicines Optimisation Teams to implement best practice. Provide expert medicines advice to GPs, nurses, and other PCN clinicians. Support patients and carers with medicines-related questions and promote shared decision-making. Public Health and Education Deliver training sessions and updates for practice and PCN staff on safe prescribing and medicines use. Promote patient education around medicines, adherence, and lifestyle interventions. Professional Responsibilities Maintain professional registration with the General Pharmaceutical Council (GPhC). Participate in clinical supervision, peer review, and CPD to maintain and enhance clinical competence. Work within professional and organisational policies, ensuring confidentiality and data protection compliance. Contribute to the supervision and mentoring of pharmacy technicians, trainees, or students where appropriate.
