The Role The role of Home Support co-ordinator will include the supervision of a dispersed team of Home Support Workers who work across the borough providing reablement services to people within their own homes and working with a range of internal and external partners to achieve the best possible outcomes for individuals. With the aim of supporting individuals to retain, regain and develop the skills to manage their lives and environment. The role will include the safe recruitment and selection of workers and their retention in care services. You will allocate and monitor the progress and quality of work in your area of responsibility. As well as assisting and contributing to the development of risk management plans to support individuals independence within their home. You will contribute to care plans and their review and the protection of individuals from harm and abuse. About You The Co-ordinator role assists with the screening and allocation of packages of support on behalf of the Home Based Reablement Service, this involves working with computer-based information systems. These systems support the rostering and planning of the work that is required. You will need to be physically fit and able to carry out aspects of the job such as lifting/carrying/standing for lengthy periods. You must ensure that the service is delivered to the highest achievable professional standards to support vulnerable adults with both rehabilitative and / or mental health needs to retain / regain community living. You will have Knowledge and understanding of relevant legislation and current developments affecting adults/older adults. Time keeping skills are essential for this role. The service operates over 365 days per year between the hours 7.00am 10.00pm therefore the post holder will be required to work flexibly on a 5 from 7 basis including evenings, weekends, and Bank Holidays.
