We are redesigning our community services model and implementing a new Intermediate Care Team. An exciting opportunity has arisen to employ Nurses to deliver urgent community response services (providing care from 8am to 9pm, 7 days a week). These nurses will play a crucial role in providing essential support and care to individuals, focusing on promoting independence, dignity, and well-being. This role is ideal for someone wishing to progress their career in nursing.
The objective is to optimise safe and timely discharge from inpatient stays for patients requiring continuing care or to prevent admission through interventions manageable at home.
The post holder will work closely with medical and therapy colleagues as part of the Intermediate Care (Hospital@Home) Service, providing comprehensive multidisciplinary care for adults.
They will support the Team Leader and Deputy in developing and implementing the service, including facilitating integrated working with colleagues in Local Authorities and third sector organisations to support individuals within their homes where appropriate.
The post holder will take delegated responsibility from the Team Leaders/deputy for managing patients across various settings, providing timely clinical assessments and treatment plans.
Welsh Skills (Desirable): Speaking and listening skills at Level 3 (CEFR B2 or above) are considered an advantage but are not essential.
