Avalon is currently supporting a Supported Housing Provider in Northampton to recruit a Supported Housing Coordinator.
The Opportunity:
The successful candidate will be responsible for supporting individuals with mental health and substance misuse and those who are at risk of homelessness.
The successful candidate will:
* Open and close premises – conducting checks
* Liaising, dealing and meeting with clients family members, Mental Health Services, social workers, CPN’s, GP’s, District Nurses, Hospital staff/wards, The coroner and coroner’s office, emergency services, police, probation, pharmacies, psychiatrists, appointees etc.
* Supporting clients with their finances and liaising with financial appointees within the Council.
* Update annual support plans, risk assessments and personal information as and when required
* Carry out bi-yearly staff supervisions.
* Carry out quarterly staff meetings.
* Dealing with clients and visitors at the door, rent payments, complaints, questions, repair issues, financial issues etc
* Booking, managing and attending social care appointments and reviews.
* Book and attend external client reviews
* Keeping up to date records for all health and safety to include gas certificates, fire detection, Electrical and emergency lighting, staff first aid.
* Create and manage staff rotas.
* Liaise with on site and source and deal with external maintenance for housing related repairs.
* Dealing with client Safeguarding issues.
* Liaising with the Local council for housing related issues and move on reports.

* £26,000 - £30,000 per annum
Requirements:
* Enhanced DBS
* Previous experience working within supported Housing
