Key Responsibilities:

* Undertake assessments of adults and their carers, identifying needs, risks, and personal budgets.
* Develop, review, and implement support plans, ensuring services remain person-centred and outcome-focused.
* Ensure people are supported to make their own decisions, promoting choice and control in how they receive care.
* Work autonomously to manage a caseload, prioritising work within eligibility criteria and resource availability.
* Undertake risk assessments and take responsibility for actions in situations where a person’s safety or liberty is at risk.
* Apply safeguarding policies and procedures to protect adults at risk of abuse, neglect, or exploitation.
* Ensure compliance with the Data Protection Act 1998, GDPR, and information-sharing protocols.
* Work collaboratively with health, community, and voluntary sector partners to integrate support services.
* Provide advice and supervision to other social care practitioners and contribute to the development of best practice.
* Support the training and supervision of student social workers.
* Maintain an up-to-date knowledge of relevant legislation, policy frameworks, and best practices.
* Apply advanced knowledge of safeguarding, mental capacity, and positive risk-taking.
* Undertake additional duties, such as acting as an Approved Mental Health Professional (AMHP), Best Interest Assessor (BIA), or Practice Assessor, as agreed.

Qualifications and Experience Required:

* Degree in Social Work (or equivalent) from a UK university.
* Registration with Social Work England.
* Post-qualifying experience in adult social care, mental capacity assessments, safeguarding, or integrated health and social care.
* Knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 2008.
* Strong assessment, case management, and decision-making skills.
* Ability to work independently while contributing to a multi-agency team.
* Experience in providing advice, mentoring, and supervision to social care staff and students.
