Department: Long Term Care
Location: Edna Tina Wilson Living Center
Hours Per Week: 20
Schedule: Evenings/Nights (3 PM - 11 PM and 11 PM - 7 AM)
About the Role
Are you a compassionate and dedicated Licensed Practical Nurse looking to make a real difference in the lives of long-term and post-acute care residents? In this role, you will not only provide professional patient care but also lead and support a team of Certified Nursing Assistants (CNAs). Your leadership will be vital in ensuring the highest quality of care and accurate documentation.
Your Responsibilities
* Administer medications accurately and on time, ensuring all documentation is completed.
* Perform treatments as per physician orders and facility protocols, documenting progress diligently.
* Maintain concise and complete resident care documentation using our computerized medical record system.
* Communicate effectively with staff through written and verbal reports, adhering to departmental protocols.
* Monitor residents closely and report any changes in condition to the charge nurse or physician promptly.
* Conduct pre-reviews of medication administration records as per facility guidelines.
* Complete required MDS documentation and related observations in a timely manner.
* Support the CNA team by assisting with direct Activities of Daily Living (ADL) care for residents.
What We’re Looking For
Required Qualifications
* Graduation from an accredited School of Practical Nursing.
* Current licensure in the State of New York.
Preferred Qualifications
* One year of experience in a similar role is preferred.
Education & Certifications
* BLS - Basic Life Support (American Heart Association)
* LPN - Licensed Practical Nurse (New York State Education Department)
Physical Requirements
This role involves medium work, requiring the ability to exert 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently. You will need to be comfortable with frequent walking, standing, or squatting.
