Responsibilities
* Support and accompany clients to essential appointments (health, debts, housing, etc).
* Advise clients to access information on housing, health, welfare, benefits and other resources.
* Provide medication support where required.
* Actively engage residents in developing the service.
* Help assess potential new clients.
* Liaise with external agencies including health professionals.
* Manage incidents or challenging behaviour.
* Develop person-centred Support Plans and Risk Management Plans and keep accurate records.
About You
* Working knowledge of the needs and support requirements of people with complex mental health needs, or experience in another social care setting.
* Great communicator, building trusting and professional relationships.
* Passionate about making a difference and thrive in a fast‑paced environment.
* High level of resilience.
* Empower people to develop their skills, strengths and talents.
* Good knowledge of community resources and organisations.
* Comfortable dealing with difficult or complex situations.
* Experience that includes mental health support, education, training, or leading activities.
* Ability to participate in our 24‑hour shift rota system and on‑call rota.
