Responsibilities
* Work closely with the multidisciplinary team to promote clinical and cost‑effective prescribing through evidence‑based guidelines.
* Develop, audit and monitor medicines management processes within the service.
* Provide training and supervision to departmental staff.
* Lead services in Frailty and Care of the Older Person, collaborating with Specialist Lead Pharmacists.
Qualifications and Requirements
* Strong organisational and communication skills.
* Previous experience in a clinical pharmacy environment.
* Completed a post‑graduate diploma in clinical pharmacy.
* Evidence of multi‑disciplinary teamwork is advantageous.
* Commitment to continuing professional development.
* Possession of a prescribing qualification or be prepared to work towards one.
Training and support will be provided.
