Overview
An exciting opportunity for an experienced HCSW to join the St Andrews inpatient rehabilitation team. The Rehabilitation Service provides rehabilitation to the adult population within the two wards of the community hospital, with some patients followed up at home or as an outpatient.
Responsibilities
* Work with the Physiotherapy Service to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge.
* Carry out ongoing treatment programmes for a mixed caseload of mainly older people as part of a multidisciplinary team.
* Collaborate with physiotherapists who provide specialist assessments and allocate work.
* Attend regular supervision, personal development planning (PDP) and training opportunities.
* Be prepared to work at other community hospital sites when required.
Qualifications & Requirements
* SVQ Level III in Health and Social Care or equivalent.
* Experience as a Physiotherapy and/or Occupational Therapy support worker (desirable).
* Experience working with older people (desirable).
* Good organisational and communication skills.
* Ability to work independently and within a team.
* Ability to travel throughout the area.
* Public holiday working is in operation.
