About the role
As a Community Care Officer (CCO) level 2 you will support adults with care and support needs across Lincolnshire, ensuring they receive the right support to live independently. Your day‑to‑day work will include completing Adult Needs Assessments and Reviews in line with the Care Act2014 and delivering statutory duties in a person‑centred, strengths‑based approach.
* Conduct Adult Needs Assessments and Reviews in line with the Care Act2014.
* Deliver statutory duties under the Care Act2014.
* Use a person‑centred, strengths‑based approach to support independence.
* Travel efficiently within the Lincolnshire Conversation areas, including unplanned visits outside the local area or county.
* Maintain a mixture of home‑based and office work, with a minimum of two days in the office per week.
* Stay available for unplanned visits at short notice when required.
About You
* Experience in strength‑based assessment, risk assessment and care planning.
* Self‑motivated, proactive and enthusiastic, with a positive attitude.
* Respectful of individuals, ensuring dignity and choice at the heart of all work.
* Excellent communication skills – verbal and non‑verbal – and professional curiosity.
* Creative thinking and problem‑solving ability to deliver innovative solutions tailored to personal needs.
