Key Responsibilities
* To assess and develop personalised care plans for individuals admitted onto the ward.
* To (where appropriate) complete or delegate visits to individuals' homes.
* To provide clinical leadership and supervision to members of the therapy team.
* To work with partner organisations in order to promote a 'discharge to assess' model of care.
* To work alongside the therapy leads in order to innovate and develop the therapy service.
* To demonstrate a commitment to personal development, recognising that the increasing demand for flexibility in working practices is essential. Flexible working options such as part-time working, compressed hours, job shares, and hybrid working models enable staff to balance personal and work commitments, while supportive arrangements can be made when they align with service needs and benefit other team members.
* Location: Johnson Community Hospital, Spalding Road, Pinchbeck, PE11 3DT.
