Single Point of Contact (SPoC) Clinical Practitioner
At Prospect Hospice, we believe that everyonedeserves outstanding, compassionate care at the end of life. Our Single Pointof Contact (SPoC) Team is the first point of contact for patients, families andhealthcare professionals seeking specialist palliative care advice and support.
We are looking for anexperienced and motivated SPoC Clinical Practitioner to join this innovativeteam providing expert clinical triage, holistic assessment and timelyguidance to ensure that patients receive the right care, at the right time.
This is a dynamic and patient-facing role ideal for someone who thrives on variety, clinical responsibility, and meaningful impact.
Hours: 15 hours per week, Monday to Friday between 8am and 6pm
Main duties of the job
As a SPoC Clinical Practitioner, you will play a vital role in providing a central, telephone-based access point for hospice care and clinical advice.
You will:
* Receive and assess the urgency of hospice referrals and clinical enquiries.
* Provide clinical advice and emotional support through the advice line by responding to calls and offering high-quality, evidence-based guidance to patients, carers, and healthcare professionals.
* Maintain accurate and confidential patient records.
* Work collaboratively with multidisciplinary teams across primary and secondary care.
* Participate in clinical governance, audits, and service improvement projects.
* Contribute to training and education of internal and external health and social care professionals.
* Support advance and anticipatory care planning, always keeping the patient at the centre of decision-making.
What we are looking for;
You will be a Registered Nurse or Allied Health Professional with the relevant years post-registration experience, ideally with knowledge or experience in palliative care, community nursing, or acute care settings.
You will bring:
* NMC or HCPC registration (e.g., Registered Nurse, Paramedic, or Allied Health Professional).
* Excellent communication and triage skills.
* Confidence in clinical decision-making and risk assessment.
* A compassionate, patient-centred approach.

Our Team
The Single Point of Contact Team is a highly skilled, innovative and adaptable team ofhealth care professionals who are the first point of contact for all patients,family members and health care professionals who require specialist palliativeadvice, holistic patient needs assessment and support with advance careplanning. We work with other health careprofessionals in primary and secondary care to provide oversight and guidance, to ensure best practice for patientsand families, keeping the patient at the centre of care and allowing themautonomy in decision making.
Main Purpose & Scope
TheSingle point of Contact (SPoC) Clinical Practitioner is responsible forsupporting the receiving and triaging of referrals and requests for clinicaladvice, made to the hospice. The prime purpose is to provide patients, carers, the public and health and social careprofessionals with a telephone based central point of access for hospice careand advice.
A key element will be to ensure a timely responsive service, ensuringthat all clinical referrals to Prospect Hospice are triaged and actioned as peragreed protocols.
Anotheraspect will be to respond tocalls to the advice line, offering quality, professional advice and support topatients and healthcare professionals, signposting them as appropriate to otherrelevant services.

Person Specification
Experience
* Relevant years post registration experience.
* Experience in palliative care.
* Understanding of clinical governance.
* Experience in community care.
* To have an understanding and adequate knowledge base regarding effective symptom control management within palliative care and confidence to make safe recommendations for symptom control.

Qualifications
* NMC Registered Nurse or Allied Healthcare Professional.
* Evidence of ongoing professional development within palliative care and willingness to undertake further study.
* Diploma/Degree in Palliative Care/Teaching qualification.
* Demonstrate the ability and willingness to complete relevant level 6/7 studies. This would include Patient Communication course, complexities of end-of-life care and remote clinical decision making.
Skills
* IT Literacy/Experience of MS packages.
* Ability to make decisions, organise and prioritise.
* Ability to demonstrate motivation and innovation.
* Ability to work independently and as part of the MDT.
* Ability to work alongside and support other members of the team.
