Overview
An exciting opportunity for an enthusiastic and dedicated person to be part of the Waltham Forest Rehabilitation Service, a multidisciplinary team providing short term intensive rehabilitation to patients in their own homes following illness or injury.
You will work with the multidisciplinary team to provide patient-centred rehabilitation programmes and support patients to regain independence after discharge from hospital.
Responsibilities
* Deliver short term intensive rehabilitation in patients' homes in collaboration with physiotherapists, occupational therapists, rehabilitation assistants and rehabilitation practitioners.
* Collaborate with the multidisciplinary team to develop and implement patient-centred rehabilitation programmes.
* Travel to meet the needs of the role and provide services in the community.
* Promote independence and improve function for people recently discharged from hospital.
Qualifications and Requirements
* Relevant experience in a health and social care setting and base level of occupational therapy/physiotherapy knowledge.
* Knowledge of patient/client conditions relevant to community rehabilitation.
* Ability to work under own direction.
* Willingness and ability to travel to meet the needs of the role.

* Induction: New starters join the Trust on the first Monday of the month and undertake a comprehensive induction (up to two weeks) including mandatory and systems training.
* Location/Induction Details: Induction held at the Trust head office in Rainham, Essex.
* High Cost Area Supplement Outer London: 15% of basic salary (minimum £4,551; maximum £5,735), pro rata for part-time staff.
