Job overview

The post holder will be a highly specialised inherited cardiac conditions (ICC) practitioner, who will work as part of the ICC nursing and mutidisciplinary team .

The post holder will facilitate family screening, clinical review and they will ensure individuals are cared for in the most appropriate setting utilising their clinical expertise, leadership, management and education skills to ensure seamless care from diagnosis to end of life. They will provide advice, support and information from the time of referral and throughout the care pathway.

The post holder functions as part of the wider multidisciplinary (MDT) team in hospital and the community t o provide advice, information and support to patients and their families affected or at risk of ICC’s including, cardiomyopathy, inherited arrhythmia and aortopathies. This will include supporting patients with ongoing treatment, and the development of nurse led clinics and pathways to monitor management and optimise treatment strategies, including mavacamtem.

Applicants who do not meet the full criteria but demonstrate a willingness to work towards achieving the enhanced practitioner level may be offered a Trainee Band 6 Practitioner position until the Band 7 requirements are met.


Main duties of the job

Contribute towards sharing good practice within the multidisciplinary team.

· Line management of Band 3 Support Secretary/Mavacamten Coordinator

· LVOTO/Mavacamten MDT attendance & support (covering when the coordinator is absent)

· Prescribing Mavacamten for established patients (6-monthly follow-ups, prescription-only when no concurrent appointment)

· Ad hoc reviews and helpline support (potentially shared with ICC Band 6)

· Work cohesively with all members of the multidisciplinary team and promote effective liaison within the team ensuring the very best service to patients is provided at all times.

· Work specifically with adult and paediatric colleagues in ICC services as well as arrhythmia service and genetic services.

· Act as an advocate for the person and the family enabling them to express their needs effectively when dealing with the MDT team and communicating back to the family. Evaluate and respond specifically to any disabilities, emotional and or additional learning needs.

Detailed job description and main responsibilities

· To provide specialist nursing advice and support to patients, families and carers from the time of referral and throughout treatment and follow-up phases of treatment and care.

· To lead the development, improvement, management and promotion of the service ensuring the delivery of high-quality, cost-effective care.

· To lead on the implementation and monitoring of adherence to relevant clinical guidelines.

· The Post holder will participate in nurse-led inherited cardiac condition clinics in accordance with the developing needs of the service.

· The post holder will work with the existing ICC MDT to deliver treatment (including mavacamtem) montoring and titration through clinics and the nurse helpline.

· To use their expert knowledge and clinical judgement to decide whether to refer inherited cardiac conditions patients for investigations and make diagnoses within own scope of practice.

Qualifications


Essential criteria

* RGN.1st Level with current NMC registration
* Evidence of relevant education level 3 or M level post registration
* Evidence of master’s level academic study or willingness to work towards Masters (in relevant health related subject)
* PACR/PADRAP, Academic or wilingness to work toward


Desirable criteria

* Genetics/Genomics courses including genetics counselling courses training
* Registered non-medical prescriber


Knowledge and Experience


Essential criteria

* Substantial experience of caring for adult patients with cardiac disease and providing specialist nursing care
* Thorough and up to date knowledge of nursing theory and best practice particularly within speciality area
* Experience of running nurse led clinics and management of patient caseload


Desirable criteria

* Recent ICC & genomics experience, showing in depth knowledge of ICC’s and/or inheritance risks and genetic implications
* Experience of working with bereaved families
* Experience of research, data collection and audit skills to improve/develop clinical practice
