Job Opportunity: Social Worker – Transition Team (Lifelong Services)

Location: Countywide – Offices based in Chichester and Worthing
Contract: Temporary | 19 weeks | Full-time (37 hrs/week)
Start Date: 3 November 2025
Rate: PAYE £28.03/hr | Umbrella/Ltd £37.54/hr
IR35 Status: Inside IR35
West Sussex County Council is seeking a dedicated and experienced Social Worker to join our county-wide Transition Team, supporting young people aged 16+ with lifelong disabilities—including physical disabilities, learning disabilities, and autism—as they move from childhood into adulthood.
Responsibilities
* Undertake high-quality assessments and reviews, including Mental Capacity Act assessments.
* Respond to safeguarding enquiries and contribute to multi-agency planning.
* Support individuals and families through person-centred planning and creative problem-solving.
* Build strong relationships with health professionals, support providers, and partner agencies.
* Contribute to the duty function and manage incoming work under supervision.
* Maintain accurate records and comply with Social Work England training requirements.
What You’ll Bring
* Qualified Social Worker with experience in learning disabilities and transitions.
* Strong understanding of safeguarding, risk enablement, and legislative frameworks.
* Ability to work independently and flexibly across the county.
* Excellent communication, assessment, and planning skills.
* Commitment to evidence-based practice and continuous professional development.
Working Arrangements
Countywide travel required for assessments and face-to-face work.
Office-based induction at Chichester or Worthing, followed by hybrid working (up to 2 days/week in office).
