As a Community Mental Health Practitioner, you will coordinate your own caseload, have the opportunity to provide a range of specialist mental health assessments and evidence based recovery focused interventions. You will work collaboratively as part of a Multi-Disciplinary Team with people with severe and enduring mental health difficulties and complex presentations, their carers and other agencies, putting the person at the centre of all you do enabling them to access mental health specialist support when and where they need it.

Main duties of the job
To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.
To manage caseload and service delivery in accordance with and in order to contribute towards the attainment of Service/Trust productivity requirements.
Work with individuals to assess their mental health, recognise mental illness and identify their related needs and circumstances; and enable them to understand, manage and where appropriate change their behaviour.
Plan, implement, review and improve interventions to meet people\'s identified needs and manage their inherent risk.
To assess carer and family\'s needs and develop, implement and review programs of support for carers and families.
To maintain accurate and timely clinical records.
To adhere to codes of professional conduct and ethics, as well as associated legislation.
To develop clinical practice having due regard to NICE guidelines and other best practice guidance.

Person Specification
Experience (Essential)
* Relevant experience of working with adults with mental health needs or substantial experience of working with individuals in crisis and skills transferable to community/in-patient settings.
* Experience of working with people with severe and enduring mental illnesses
* Evidence of continuing professional development
Skills and Competences (Essential)
* Sound knowledge of national agenda for mental health
* Sound knowledge of clinical/risk assessment and understanding of information governance principles
* Delegation whilst maintaining overall responsibility for patient care, where appropriate
* Ability to lead a clinical team
* Ability to work independently and collectively
* Highly developed clinical reasoning skills
* Highly motivated and able to engage with service users and carers to improve outcomes

Qualifications (Essential)
* Recognised Professional Qualification in Registered Nursing (RMN, RLDN), Social Worker (CQSW, DipSW), Occupational Therapy RVN or RNLD or RN: 1st Level Registration (NMC) or degree or equivalent
* ENB998 / FliPS or equivalent
* Mentorship Module (degree level)
* Evidence of specialised continued professional training (degree level) in clinical practice
* Registered with a professional body (NMC, UKCP, HCPC, BACP)

* Management training
