About the role
You will join our specialist team to support adults living in their own homes, helping them maintain independence and choice. The role is short‑term, over a seven‑day period, and focuses on acute physical health crises or sudden care breakdowns. Service aims to enable safe discharges from hospital and prevent admission to residential or nursing homes.
Responsibilities
* Provide high quality care to housebound patients in Paignton and Brixham as part of a multi‑disciplinary team.
* Deliver complex, sensitive and contentious information using empathy and reassurance skills.
* Offer motivational and negotiation support where consent or cooperation may be challenging, including best interest decision making.
* Communicate timely and effectively with the team manager and other team members on professional matters.
* Liaise with patients and relatives who may be unpredictable or challenging due to stress‑related problems.
* Assist with specialist nursing and continuing care assessments where appropriate training is available.
