Overview
Advanced Clinical Pharmacist – Emergency Department. Fixed term (12 months) position based at Whiston Hospital, covering maternity leave. Part of the Emergency Department (ED) Pharmacy team at Mersey and West Lancashire Teaching Hospitals NHS Trust, providing a clinical pharmacy service to ED patients at Whiston site. This role forms part of a 7-day service; weekend, late-night, on-call, and rostered duties are required.

Responsibilities
* Lead and deliver high quality clinical pharmacy services to the designated speciality, including development of an advanced pharmacist role.
* Develop competencies and provide ward-based clinical teaching to band 6/7 pharmacists & pre-registration pharmacists.
* Practice as a Trust approved Independent Non Medical Prescriber.
* Participate in clinical audit and practice research relevant to medical specialties.
* Participate in the department rota to provide a pharmacy service over 7 days; assist in clinical training, supervision, guidelines and policies.
* Apply professional judgement to clinical, legal and ethical issues; stay informed on national policies, NICE guidance and medicines management; contribute to formulary and policy amendments as appropriate.
* Scan national medical/pharmaceutical information to identify developments affecting patient care.
Clinical & Professional Responsibilities
* Provide an advanced clinical pharmacy service to specified wards in accordance with Trust policies; take drug histories on admission; review patient notes for diagnoses and test results to ensure safe and effective medicines use.
* Ensure appropriate indications and therapy; assess safety of existing therapies; advise on peri-operative medications; monitor for adverse drug reactions and perform therapeutic drug monitoring where appropriate.
* Ensure formulary and policy compliance; educate patients; develop evidence-based treatment protocols; advise on off-label drug use; monitor stock levels; participate in consultant ward rounds and multi-disciplinary meetings.
* Provide information to medical/nursing staff, GPs and other staff; liaise with primary care to obtain accurate drug histories; record clinical interventions; report drug errors; contribute to drug monitoring services and care across primary/secondary interfaces.
* Check prescriptions, handle controlled drugs in accordance with the Misuse of Drugs Act, supervise pre-registration pharmacists, and ensure safe custody/destruction of drugs.
* Support recruitment, training, and performance development; participate in CPD and Professional Revalidation as per GPhC guidelines; comply with Health and Safety and COSHH regulations.
Medicines Optimisation
* Develop and implement Medicines Optimisation Strategy; contribute to evidence-based prescribing guidance; horizon-scan to identify future developments in medicines impacting the Trust and local health economy.
Qualifications
* Essential: Vocational master’s degree in pharmacy; Postgraduate Diploma/MSc in Clinical Pharmacy; 1 year pre-registration training; General Pharmaceutical Council registration; Non-Medical prescribing qualification or working towards; commitment to continuing professional development/education.
* Desirable: Membership of the Royal Pharmaceutical Society (RPS).
Experience & Skills
* Essential: Extensive clinical pharmacy experience across multiple specialties in an acute hospital; strong clinical knowledge of patient management; experience in a busy dispensary; experience with drug reporting and financial management; demonstrated ability to conduct clinical audit and practice research; excellent communication skills; ability to work autonomously or in a team; strong organisational skills and ability to work under pressure.
* Desirable: Experience leading/auditing or supervising audit projects; experience using Pharmacy Computer System and electronic prescribing.
