What you will do
* Responsible for supporting the delivery of health and social care functions for adults and carers in Wirral.
* Play a key role in supporting the Personalisation and Inclusion Pathways for adults and carers, ensuring the Care Act 2014’s legal framework is embedded effectively.
* Assess, access, and implement a range of social care services, providing guidance, advice, and information for adults with acute or complex long-term physical and mental health needs, including older people and carers.
* Provide supervision and practice support to AYSE (Assessed & Supported Year in Employment) and newly qualified social workers, as well as Care Navigators within the service.
Requirements
* Degree or equivalent in social work is essential.
* Current Social Work England registration.
* Significant frontline experience in Social Services within the UK, working as a qualified Social Worker.
