Job summary 1x 37.5 hours KirkleesIntensive Home Based Treatment Team is looking to recruit an enthusiasticindividual to join our existing team as a Band 6 Practitioner. The service offers rapid assessment and intensivehome-based treatment as an alternative to hospital admission, allowing serviceusers and carers to be supported in the least restrictive environment. Thisflexible, intensive and assertive approach to engagement aims to promoterecovery, co-production, inclusion and resilience. The team is able to offer arange of focused interventions, and colleagues have qualifications/experiencein the areas of Occupational Therapy, Clinical Psychology, Social Work, Medicineand Nursing. Applicants should be registered mental health nurses,Social Worker or Occupational Therapist with experience at Band 5 or above in amental health service for adults and meet the essential requirements for thepost in the person specification. Cardriver is essential to this post. A period of induction and preceptorship willbe available for the successful candidate. Main duties of the job The IHBT provides intensive home based treatment 7 days aweek, 24 hour working as an alternative to hospital admission. The post holder will be responsible for the triage ofreferrals and be responsible for assessments, care-planning and co-ordinationof care delivery within a team approach. You should be forward thinking, proactive and it isessential that you are willing to work flexibly as part of a 24 hour service,which includes shifts and working weekends. The role provides the opportunityto work across the health & social care spectrum as part of an integratedteam which has links to a multi-disciplinary service. The ability to formeffective professional working relationships with your team colleagues andservice users is an essential requirement of the job. You will be adaptable, committed, wellorganised and able to work well under pressure, along with an ability todemonstrate leadership skills and deliver supervision to colleagues. Job description Job responsibilities Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multi-disciplinary team as part of the Intensive Home Based Treatment Team and in the role of Senior Mental Health Practitioner for the Mental Health Liaison Service (if applicable to the business continuity of services). Management of team caseload in line with C.P.A. procedures and risk management protocols. Provision of leadership to the Intensive Home Based Treatment Team in respect of psycho-social interventions to people and their carers in line with national and local practice standards e.g. NICE. Supervision and support of other team members. Participation in service development and clinical governance structures. Assesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action. Advises and supports the person and carers to promote health and well-being and to prevent illness. Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required. Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community. Functions as a member of a multi-disciplinary team. Management of enquiries and direct referrals of people in accordance with Enhanced team operational Policy and participates in the team 24 hour duty rota including on call system (if applicable to the business continuity of the service). Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements. Responsible for acting up in the absence of the team leader Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency. Participates in the formulation of policies at unit level. Maintains accurate mileage records. Manages caseload with due economy of cost and time. Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only). Responsible for the ordering of stock with due economy.
