Job Role
We are looking for a highly motivated and enthusiastic Health Care Support Worker to join our Intermediate Care Team! The Intermediate Care Service supports medical and surgical service users from discharge from hospital, to their journey home. Every day there is something different from supporting service users to undertake their personalised therapy plan, to monitoring deteriorating patients and so much more in between. If you are interested in skill and experience development and thrive on a service user journey, we would love to hear from you!
The Team boasts a variety of skills and experiences to enhance your knowledge and professional development within a supportive environment. We pride ourselves on excellent standards of care and service user satisfaction. The team works collaboratively with the multi-disciplinary team comprising of GPs, Physician Associates, Registered Nurses, Occupational Therapists, Physiotherapists, Pharmacists, Healthcare Support Workers and our Kirklees Care Team. Together we rehabilitate service users to succeed in their home environment, preventing hospital admissions and preserving safety.
