We have an exciting opportunity for a Recovery Coordinator who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs. With this role you’ll be working as part of the Coventry team, based in our Change Grow Live hub and community venues.
This is an exciting and unique opportunity to be part of the Community Respiratory Clinic, an innovative project funded by the Integrated Care Board (ICB). The project is paving the way for tackling health inequalities and bringing care to the most vulnerable in society. By delivering care to individuals who often do not engage with primary or secondary healthcare, we aim to increase early diagnosis of respiratory conditions and reduce drug-related deaths.
You’ll work closely with nurses, clinicians, recovery coordinators, and partner organisations to ensure individuals receive the right support at the right time. From taking vital signs to supporting referrals into primary and secondary care, your work will be hands-on and impactful.
Location Coventry
Full Time Hours 37.5 per week
Full Time Salary £27,861.26 - £32,002.35 dependent on experience, pro rata for part time hours
Contract Type Fixed term for 24 months from start date
* Please note Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
As a Healthcare Connector, you’ll play a vital role in promoting healthier lifestyles and improving access to healthcare for our service users. You’ll coordinate and support the delivery of health and wellbeing interventions, including clinics and screenings, helping individuals overcome barriers to accessing care and addressing health inequalities.
About the role
* Coordinate, and support the day-to-day delivery of health and wellbeing interventions and health clinics at the Change Grow Live Coventry hub and associated community venues.
* Support service users in accessing treatment and clinical interventions
* Ensure that service users receive high-quality health and recovery interventions
* Undertake initial physical and mental health screening for people accessing our services and ensure appropriate signposting/referral to primary care or secondary care health services for identified health issues
* Following training and competency sign-off undertake interventions such as blood pressure, pulse, temperature monitoring, height and weight recording, and ECGs for service users as indicated or requested
About you
* Have a strong understanding of substance misuse issues and experience of working within a similar field
* Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour.
* Be an excellent communicator, both verbal and written with good IT skills
* Proactive and able to work on own initiative with excellent time management and prioritising skills
* Have a good working knowledge of mental health interventions, services, and good practice
* Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
* Ability to manage change successfully in a way that prioritises the needs of service users
* Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
* Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
