Social Worker - Rehab Access
The role will involve formulating specialist assessments for individuals that require rehabilitation for both community and inpatient settings, and working within the Mental Health Intensive Support Team. It requires a qualified social worker registered with Social Work England, a passion for mental health, and a commitment to professional development.
Key responsibilities include: establishing and conducting specialist assessments, coordinating rehabilitation plans, ensuring risk management and safeguarding, and collaborating with multidisciplinary teams to promote independence and wellbeing for vulnerable adults and their caregivers.
Travel is an essential part of this job; candidates must have reliable transport.
Qualifications
* Social Work qualification (e.g., Degree in Social Work, DipSW or equivalent).
* Registered with Social Work England and able to evidence registration.
* Evidence of continuing professional development in line with the Practitioner Development Framework.
* Completion of the Practice Educator Award.
* Relevant post‑qualifying or vocational training courses.
Knowledge and Expertise
* Demonstrated practice supporting the core principles of the College of Social Work and the Families and Wellbeing Directorate.
* Knowledge of relevant legislation, including the Mental Health Act 1983, Mental Capacity Act 2005, Equality Act 2010, Care Act 2014, and Human Rights Act 1998.
* Understanding of confidentiality, data protection and safeguarding procedures.
* Effective verbal and written communication skills with families, professionals and community agencies.
* Awareness of current practice issues and challenges within social work, and commitment to anti‑discriminatory practice.
* Ability to work collaboratively with a range of carer groups, families and partner agencies.
* Willingness to relocate across teams as service needs arise.
* Sound assessment, interviewing and IT skills (e.g., Microsoft Office).
* Demonstrated emotional intelligence and resilience related to the role.
Experience
* Proven experience of working with carers, vulnerable individuals and families.
* Experience arranging support plans, packages of care and working with partner agencies.
* Experience working within a local authority or similar multi‑agency setting.
* Experience facilitating choice and control for adults and carers in person‑centred practice.
