What the Job Involves As a Social Work Assistant located at County Hall within the Adult Services Review Team, you will collaborate closely with the Team Manager, Social Workers, and Duty Workers. Your responsibilities include arranging and monitoring services as per individual care plans, offering advice and practical help, liaising with various agencies, completing care plan reviews and reassessments, participating in team meetings, and adhering to Council policies. You will contribute to promoting equality and ensuring health and safety, with a flexible approach to work locations and times as needed. Skills Educated to GCSE standard or equivalent NVQ/GNVQ Level 3 or 4 in Social Care Experience with older people and adults with physical disabilities Experience in working with various agencies Ability to build relationships with service users and carers Good communication and organizational skills Administrative abilities Full Valid Driving Licence with sole access to a vehicle during working hours
