Minimum 1+ years nursing experience required.
We are seeking an experienced and dedicated Registered Nurse (RN) to join our team on a contract basis for a client in Bainbridge. If you are passionate about patient care and have a strong background in home health, we would love to hear from you! (OASIS experience is required)
Key Responsibilities:
* Provide comprehensive nursing care to patients in their homes.
* Perform initial and ongoing patient assessments, including vital signs, medication management, and wound care.
* Develop and implement individualized care plans based on patient needs.
* Educate patients and their families on disease management, treatment plans, and preventive care.
* Collaborate with physicians, therapists, and other healthcare professionals to ensure a cohesive care plan.
* Document all patient care activities accurately and promptly in electronic medical records (EMR).
* Monitor and report changes in patient condition to the supervising physician.
* Ensure compliance with all state and federal regulations related to home health care.
Qualifications:
* Active RN license
* Minimum of 3 months of home health experience using OASIS
* HCHB experience is a big plus
* Strong clinical skills, including IV therapy, wound care, and medication management.
* Proficient in using electronic medical records (EMR) systems.
* Excellent communication and interpersonal skills.
* Ability to work independently and manage time effectively.
* Valid drivers license and reliable transportation.
