A Vacancy at Cheshire and Wirral Partnership NHS Foundation Trust. We are looking to recruit a full time Advanced Clinical Practitioner for Specialist Palliative Care. This exciting new role will be part of the specialist palliative care (SPC) multidisciplinary team, working in partnership with the specialist palliative medical consultants, clinical nurse specialists, SPC physio, OT and therapy assistant. There will be regular liaison with the Hospice and Hospital SPC team. This role will clinically support the SPC caseload across 8 neighbourhood teams. Working within the four pillars of advanced practice we require a specialist palliative care ACP to support in shaping the future of palliative care in West Cheshire.

Responsibilities
* Provide team leadership in driving clinical workforce development and embed general palliative care competencies across community services.
* Act as a specialist palliative care advanced practitioner with advanced clinical expertise and evidence-based approach within this field.
* Develop enhanced clinical skills within the specialist team, facilitate clinical supervision, support autonomy and advanced clinical decision making.
* Lead on the expansion of non-medical prescribing, empowering SPC clinicians and the wider workforce to deliver timely and effective symptom management.
* Champion patient safety, continuous improvement and collaborative working with system partners.
* Support driving quality, person-centred outcomes, research and innovation; focus on clinical audit, service evaluation and continuous quality improvement initiatives.
Requirements and qualifications
* Specialist palliative care advanced practitioner with experience as a non-medical prescriber.
* Advanced clinical expertise and an evidence-based approach within palliative care.
* Ability to develop enhanced clinical skills, provide supervision, and support autonomy and decision making.
* Commitment to quality improvement, clinical audit, and service evaluation.

Practical details
All new starters commence on one fixed start date each month in line with the Prepare to Care (P2C) induction programme date, which offers up to 3 weeks training to meet induction requirements.
