Overview
An opportunity has arisen for an enthusiastic, professional and experienced Practice Nurse. Stourside Medical Practice is a forward thinking 6000 patient GP practice in the centre of Halesowen. We are a Training Practice with an experienced team of clinical and non-clinical staff, including a paramedic and a First Contact Physio. We are looking for someone to work part-time, 1-2 days a week. We pay an enhanced rate for extended access shifts which are currently available to work if desired by the successful candidate.
Main duties
The successful candidate will need experience of working in a primary care setting and be able to perform standard treatment room tasks such as immunisations, cervical smears, dressings and chronic disease management.

We have an experienced team of salaried GPs and Nursing staff, and the practice actively participates in a thriving Primary Care Network with a variety of allied health professionals. We are an approved Training Practice with 2 trainees in post.

Job responsibilities
* Managing and treating long-term conditions with an emphasis on our 400 diabetes patients.
* Supporting and managing the health needs of women presenting for HRT, contraception, and cervical cytology consultations.
* Implementing and participating in vaccination and immunisation programmes for both adults and children, including patients traveling abroad, and vaccines such as Prosta, Zoladex, Inclisiran, Denosumab, RSV, shingles, influenza, Covid-19 and pneumococcal vaccination.
* Meeting the needs of patients presenting for wound care or suture removal.
* Hypertension review and management.
* Asthma and COPD reviews.
* Management of other chronic diseases.
* Raising safeguarding concerns when needed.
* Maintaining a clean, safe environment, adhering to hygiene and infection control.
* Providing home visits for housebound patients needing support for long-term conditions.
* Using a clinical protocol to file normal lab results.
* Selecting patients for, and attending, a quarterly diabetic MDT with a hospital consultant and the GP diabetic lead.
* Review of medical triage submissions.

Essential
* Ability to communicate clearly and effectively in English, both written and spoken
* Good organisational and time management skills
* Ability to work under pressure and on own initiative
* IT skills relevant to General Practice (word processing and database work)
* Commitment to quality, patient satisfaction and equal opportunities
* Team worker and adaptable to changing needs
* Experience of working in primary care or diabetes management in a hospital/community setting (5 years post-registration)
* Experience of chronic disease management and treatment room procedures
Desirable
* Knowledge of EMIS Web and Docman
* Non-medical prescribing qualification
* COPD Diploma
* Diabetes Diploma
Qualifications
* Relevant Nursing degree
* Current NMC registration with an active PIN
