Station Lane Medical Centre is a well established practice with a list size of approx 7,800 patients. We are a hardworking, rapidly moving, pro-active, high achieving Practice in Featherstone, Pontefract.
We are looking to recruit a Practice Nurse to join our busy and friendly team here at Station Lane Medical Centre. We would welcome applicants from community nursing or secondary care settings. We offer excellent training opportunities.
Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients.
The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
The post-holder will have the required level of training and competence and will work within patient group directions where these are available.
Main duties of the job
Chronic Disease Management: Contribute to the management of patients with chronic diseases, identified by QOF in particular, Asthma, COPD, CHD, CVD, Depression and Diabetes. Learning Disabilities and Mental Health.
Health Promotion: Address patient in order to identify problems of: Diet, Exercise, Smoking Cessation, Obesity, Alcohol, Drug misuse, Family Planning.
Health Screening : Organisation and management of health screening activities for patient Immunisations, Cervical cytology, Well person checks and Child health & Child Protection
Infection Control: Adopt infection control measures within the Practice according to the Infection Control Manual.
Administrative and professional responsibilities: Ensure accurate notes associated with patient health care are recorded in the patients notes on the clinical system.
Attend Practice meetings and participate in an annual performance review of your own personal and professional development.
Confidentiality: In the course of your duties you will have access to confidential information relating to patients, all such information is to be regarded as strictly confidential.

Management of chronic disease
1. Identifying significant abnormalities
2. Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
Wound Care and management
3. Assessing routine wounds including trauma
4. Assessing [and prescribing] to support good wound management in complex situations
Patient health checks
5. Identifying significant abnormalities
6. Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues
7. Working with patients to develop a management plan where health problems or potential health problems are identified
Risk assessment
8. Recognising issues and gathering sufficient information to refer ( drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems)
Health screening
9. Recognising issues and gathering sufficient information to refer ( womens and mens health, sexual health, older people)
10. Undertaking monitoring tasks ( smears) and providing advice as appropriate
11. Providing in-depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans.
Travel health
12. Administering injections and providing guidance in accordance with guidelines
13. Identifying problems
14. Provision of specialist and evidence-based support to the team.
Immunisation (adult and child)
