Weare looking for individuals who are willingto develop their knowledge and skills in relation to acute and communityservices. You will need to work proactively as part of thewider team to deliver the RightCare Service for the benefit of patients andpublic.
It is essential that you are motivated, enthusiastic, confident, and organised. It would suit someone with previous experience in the acuteand/or community care setting. You should have excellent interpersonal,communication, decision making and influencing skills in order to ensure thehighest standards of care are achieved. This role aims to provide comprehensiveand pro-active coordination and support, ensuring that all processes and tasksare undertaken efficiently, effectively and to a high standard, contributing tothe smooth running of the service and its overall effectiveness.
Working hours are between8am-8pm, seven days, includes weekends and bank holidays. Stafftypically work long shifts per week but other shift working options can beconsidered.
Main duties of the job
As a Patient Flow coordinator, you will be based in theRightCare department and will deal with referrals taken electronically and overthe telephone. You will be part of a wider team including Nurses,administrative officers, Social Care and Intermediate Care staff. You will sendreferrals to a range of health and social care providers, including Care Homes,you will follow up the referral to confirm service provision and let therelevant teams know.
The aim of the service is to ensure that the right care isprovided, at the right time and in the right place. You will work closely withhealth, social care teams and professionals from primary, secondary andcommunity care supporting the RightCare nursing and coordination staff access careor other more suitable alternatives to hospital admission that will meet theneeds of the service user.

Experience
* Previous experience of working in a hospital setting
* Previous experience of using Medway, Systm1 and other relevant EPR and clinical systems
