Main duties of the job

You will play a key part in supporting older adults to stay safe, independent, and well within their own homes. Your work will centre on delivering community based physiotherapy, providing personalised assessment and rehabilitation to people living with frailty. You will focus on improving mobility, strength, and confidence, helping patients reduce their risk of falls and maintain their day to day function.

You will spend much of your time out in the community, carrying out home visits, building strong therapeutic relationships, and tailoring rehabilitation to each person’s environment and goals. Alongside this, you will work closely with a supportive multidisciplinary team, contributing your physiotherapy expertise to shared care planning and proactive management of high risk individuals.

Your role will also involve supporting patients after hospital discharge, helping them regain independence and avoid readmission. You will have the autonomy to manage your own caseload, plan your visits, and make informed clinical decisions that directly influence patient outcomes.

Job responsibilities

Role Overview

The Physiotherapist will deliver home‑based assessment and rehabilitation for patients living with frailty, focusing on mobility, falls prevention and proactive management of high risk individuals. The role centres on improving mobility, strength and functional independence through targeted rehabilitation, enabling patients to remain safely at home and reducing the risk of deterioration, hospital admission and readmission.

Working as part of the Primary Care Network (PCN) multidisciplinary team (MDT), the Physiotherapist will support proactive frailty management, contributing to shared care planning and coordinated, person‑centred care across primary, community and social care services. This role plays a key part in reducing avoidable demand across primary and secondary care through early intervention and ongoing management of musculoskeletal and mobility related drivers of frailty.

Regular home visiting is required across the PCN footprint.

Key Responsibilities

* Provide specialist physiotherapy assessment and intervention including:
o Mobility, strength and balance assessments
o Frailty assessments and proactive case management
o Falls risk assessment and prevention strategies
o Home‑based mobility assessments
o Exercise programmes and mobility strategies for patients and carers
o Individualised rehabilitation programmes
o Support for safe discharge and reducing readmission risk

You will work with patients to develop person‑centred goals and care plans focused on improving quality of life, mobility, confidence and daily function.

Typical Working Pattern

* 57 home visits per day (depending on complexity and travel)
* MDT meetings and case discussions
* Time for documentation, care planning and coordination
* Proactive case management of frailty cohorts
* Care home visits and remote consultations

Multidisciplinary Working

You will collaborate with:

* GPs
* Advanced Nurse Practitioners
* Physiotherapists / First Contact Practitioners
* Social Prescribers
* Care Coordinators
* Community therapy teams
* Social care teams
* Voluntary sector services

You will contribute to MDT discussions and care planning for complex patients.

Care Home & Community Support

* Care home residents
* Recently discharged patients
* Patients identified via frailty registers or risk stratification
* Individuals requiring geriatric assessment
* Patients needing remote consultations

Service Development & Quality Improvement

* Development of physiotherapy pathways in primary care
* Audit, evaluation and QI initiatives
* MDT understanding of mobility optimisation and falls prevention
* Service evaluation and outcome reporting

Professional Responsibilities

* Maintain HCPC registration
* Work in accordance with CSP standards
* Participate in supervision and CPD
* Maintain accurate clinical records
* Follow safeguarding, governance and data protection policies

Person Specification

Essential

* Degree or MSc in Physiotherapy
* HCPC registration
* CSP membership
* Experience with older adults or frailty, including:
o Multimorbidity
o Falls risk and mobility decline
o Deconditioning / sarcopenia
o Post discharge rehab
o Community or reablement settings
* Functional assessment and rehabilitation experience
* Strong communication and MDT skills
* Ability to work autonomously in the community
* Full clean driving licence and access to a vehicle

Desirable

* Experience in primary care or community services
* Experience with care homes or frailty pathways
* Knowledge of NHS primary care systems
* Postgraduate training in frailty, rehab or advanced practice
