An exciting opportunity has come up for the Adult Community Nursing Service We are looking to appoint a Community Matron to work 3 days per week, who is passionate about high quality patient centred care to join our highly regarded Community Matron Team. The service operates from 7am until 7pm across the Twickenham borough. The post is advertised as a 6 month fixed term contract to cover a secondment within the team and is for 22.5 hours per week.

The post holder will need to provide line management and leadership for a locality nursing team, being accountable for the team caseload and the development of a skilled, competent and motivated workforce, able to meet the needs of the patients on the caseload.

Main duties of the job

To be responsible for the delivery of the locality nursing service in line with the commissioned service specification, standards, activity and outcomes, in the context of multi-disciplinary locality working.

To provide line management and leadership for a locality nursing team, being accountable for the team caseload and the development of a skilled, competent and motivated workforce, able to meet the needs of the patients on the caseload.

To work in partnership with the primary health care team, social services and the wider multi-disciplinary locality team, providing holistic assessment of health needs and the provision of nursing care to housebound people with a range of medical needs and long term conditions.

Person Specification


Qualifications

* Qualified RGN NMC - RN1 ,RNA with current NMC registration
* Nursing Diploma or BSc (Hons) Community Health (DN)
* Relevant post registration study at diploma/degree level and relevant CPD e.g. palliative care, tissue viability
* Physical Assessment Skills
* Advanced Assessment Skills
* Nurse Prescriber V100
* Mentorship Preceptorship or equivalent
* Specialist Practice Teacher/ Teaching Qualification
* Independent Non-medical Prescriber V300


Knowledge

* Understanding of the impact of Mental Health e.g. dementia, depression, on the management of medical and long term conditions
* Professional standards including confidentiality, clinical governance and NMC guidelines
* Infection prevention and control standards
* Risk assessment and management
* Knowledge of health and safety and personal safety issues
* Safeguarding Adults & Children
* Understanding of the Mental Capacity Act 2005
* Understanding of current legislation and its implications for this role, including performance indicators and targets
* Understanding of both the Health and Social Care agenda


Experience

* Experience in assessing, planning, delivering and evaluating patients in their own homes
* Experience in working independently and as part of a team
